All patients are welcome in our dental office. We are open to serve you and provide you with your individual dental needs. At Gregorich and Matack we strive for patients leaving our office smiling and confident in their care. When you are with our team of professionals, we will take care of you and will do everything we can to make sure you are comfortable and informed about every procedure.

Before Your Appointment

We want each and every visit at our dental office to be positive and successful. If you have any individual needs or questions, please be sure to give our staff a call at (218) 727-1448. We’re happy to make any necessary adjustments to make your visit comfortable.

To keep our staff and other patients safe and healthy, please contact our office at (218) 727-1448 to reschedule your appointment if you have any of the below symptoms, or within the past two weeks have been in contact with a positive COVID-19 case or have tested positive yourself.

Upon arrival at your scheduled appointment, please stay in your vehicle and call us at (218) 727-1448 to let us know of your arrival. We require facemasks to be worn inside the dental office when your cleaning or procedure is not in place, and when entering the office to apply hand sanitizer and have your temperature taken. If you are displaying any of the symptoms above, we will reschedule your appointment for a later date.

Your First Appointment

To help you prepare for and save time at your first dental visit at Gregorich and Matack Family Dental, the following forms are available to download, fill out and bring to your scheduled visit. These forms will also be available at your first visit, please arrive 10 minutes early to fill them out. If you have any individual needs or questions, please be sure to give our staff a call at (218) 727-1448. We’re happy to make any necessary adjustments to make your visit comfortable. Communications and attachments via email and over the internet is not secure, please do not email your patient forms but rather bring them to your first visit.

Cancellation Policy

We require a 24-hour notice for cancelations, please call us at (218) 727-1448. In the event you No Call/No show to an appointment, your account will be billed a $60.00 fee. Two No Call/No Shows will include a $60.00 fee as well as any further scheduling to be limited to same-day appointments. Third No Call/No Show appointment for a patient will result in the inability to schedule with our office moving forward.

    Insurance

    At Gregorich and Matack Family Dental we are committed to helping you maximize your insurance benefits. Because the complexities of insurance contracts and policies vary, we can only estimate your coverage in good faith and cannot guarantee coverage. Your estimated patient portion must be paid at the time of service. As a courtesy to our patients, we allow 45 days to render payment after the date we bill your insurance company. After 60 days, you are responsible for the entire balance to be paid-in-full. If you have any questions, our staff is always available to answer them at (218) 727-1448.
